The truth behind hair transplant safety in Turkey

Yes, Turkey offers safe hair transplants. But this answer comes with a condition. Safety relies mostly on the questions you ask yourself before booking. Turkey boasts some of the most skilled hair transplant doctors in the world. But there are also clinics that you shouldn't trust. And it's not easy to tell them apart by looking at their website.

Every now and then, there is an incident involving bad hair transplants from outside the UK which becomes the talk of town. Cases of infection, lack of hair growth despite successful grafting, missing documentation — all of these incidents happen because the patients simply had no idea how to check on the legitimacy of their procedure.

And this article seeks to fill the void created by this problem. It is penned by the people behind Lilian Health — a clinic located in Istanbul which caters to UK, European and foreign patients since 2022. From firsthand experience we have learned how to prevent everything you shouldn't do with hair transplants.

The patient that made us prove everything

Troy Samuels arrived in Istanbul from the United Kingdom. Prior to booking his surgery, he requested a Health Tourism Authorisation Certificate from us. He requested the operating license of the clinic and the qualifications of the surgeon. Troy requested a video conference call involving the three of them — he, the coordinator and the surgeon. He also asked whether the person he had been talking with before would be physically present at the surgery date in the operation theater.

This is very important because in many clinics, the person you talk to is only a sales consultant. They provide you with the information medically, but have no clinical background whatsoever. This person does not show up in the operation theater. You will meet a stranger handling your case.

In addition, Troy requested what proofs would he get after the surgery. Not an oral assurance because he knew how useless this would be. He asked for documentary proof of all grafts inserted into him.

The five-point hair transplant safety checklist

✓ Pre-Booking Safety Checklist
  1. Health Tourism Authorisation Certificate — Turkish law requires clinics serving foreign patients to have this. Request the certificate. Ensure it is valid.
  2. Clinic operating licence + surgeon's medical certificate — Ensure a licensed dermatologist or hair transplant surgeon will personally perform the operation and not merely oversee the procedure.
  3. Continuity of coordinator — You must ask: will the same coordinator speaking to me now be accompanying me on the procedure day in the clinic? If there's any hesitance, then consider it as your answer.
  4. Three-way video call — Ensure they allow you a video call where you are accompanied by both your coordinator and surgeon.
  5. Post-operative proof — not a certificate but documentary proof — What proof of my graft count will I receive after the surgery? A guarantee certificate signed with some numbers on it doesn't really mean anything. Request AI-verified video and pictures documenting each graft implanted. This practice is standard at Lilian Health — all patients receive AI Graft Counter proof after the procedure.

Graft number issues — and why this one is more important than you realize

Another very prevalent form of deception in the field comes by way of promising a certain number of grafts. This one is particularly worth knowing about since it means you could be at risk of something extremely uncomfortable happening.

For the DHI technique, the clinical ceiling in terms of the number of grafts implanted in one session is 2,800–3,500 grafts. After that point, implantation becomes too slow. Extracted grafts live in holding solution for around four hours. DHI is a slower process compared to FUE. When graft numbers exceed the time frame allowed, they die. They are subsequently implanted into the scalp in a non-viable state.

As the patient, you see your grafts immediately following surgery. In two months' time, the place where they should have been has become vacant.

In the case of Manual Sapphire FUE, a clinically feasible ceiling would be in the range of 5,000–5,500 grafts per session. If a clinic offers a significantly higher number in just one session, ask for an explanation on how they expect to keep extra grafts viable. There is no possible medical justification.

⚠ Real case we have come across

We recently did a complete hair analysis for a person from the US, and our recommendation was: 4,350 grafts with Manual Sapphire FUE to get maximum coverage on the forehead, keeping the donor site 100% natural. The other clinic he was considering offered him 7,000 grafts. He consulted us on this matter, and we told him that 7,000 hair grafts in one sitting is medically impossible — not to mention that even if it were possible, the recipient site would not be able to accommodate such density. He went ahead with the clinic offering 7,000 grafts. We hope things worked out fine for him. We don't know.

The way Lilian Health works on the very first day — and why does it matter

Once a patient arrives at the clinic, in the very first day everything goes according to the single guideline: there should be no actions performed other than relaxing.

The process starts from moving patients straight from the airport to the hotel. The patient will pay for his/her services only during the next days. The clinic doesn't perform any medical operations during the day of arrival. And we say to our patient a few words which may seem odd for a clinic: "If you want to come to the clinic right now, you can do so, see us, visit the clinic. Get to know the clinic environment and the people who work here. If even the smallest detail differs from your previous understanding, please, cancel the surgery — we will return the whole fee and the hotel costs." We haven't had a single cancelled operation since.

This isn't marketing. This is the consequence of a practice which relies on the single principle: the patient shouldn't take the clinic's word for anything. He/she must be sure about everything in advance.

And we also offer something which is not offered by very many clinics. For the prospective patients who choose it and with the permission of both sides, we introduce the previous patients from the same city or country. Not testimonials — conversations. Someone who was in the exact same situation as the patient and who will be able to give first-hand information about the exact steps he/she went through when they were there. Two or three such conversations and the anxiety regarding the trip abroad and the procedure usually fades.

ℹ Our stance on same-day operations

Some clinics in Istanbul conduct the operation on the very day the patient comes. And we strongly believe this is not the right thing to do. After coming all the way from the UK and getting into the unknown, one thing the patient needs before surgery is rest. The body functions better. The patient feels better. The surgery team will work with someone well-rested. This will cost us nothing. We would not run the operation otherwise.

The importance of proof after surgery compared to a letter of guarantee

A letter of guarantee does not hold weight without any evidence. It is impossible to check or challenge any letter saying 4,500 grafts have been put in the body. In reality, it can neither do anything nor prove anything because the expected outcome does not match reality.

Here at Lilian Health, each patient gets something different. Post-operation, our AI Graft Counter scans the entire recording of the surgery. Each graft that has been transplanted is separately counted through AI technology and video. Our patients receive the entire recording, the density map by scalp zones, and an accurate graft count of about 99%.

This accomplishes two goals. Firstly, it is real accountability — the ability of the patient to see exactly what was transplanted and where. Secondly, it sets the ground level from which recovery will start. If there is less growth in a particular area after six months, the proof shows how many grafts have been planted there, making the discussion based on facts rather than approximations.

When Troy Samuels asked us for the type of proof that he could expect, this is what we told him. Do the same when talking to any other clinic.
